Avesis has replaced the TIN & PIN combination, which providers have been using to log into the system,  with a username and password. In order to implement our enhanced security measures, your initial user experience will include an extra step, but it’s one that is necessary.

Please review the Provider Portal FAQs before you begin.  To begin, click “Sign In.” You will log in with your TIN & PIN to make the changes. And just as you do now, if you have more than one location, you will need to create a unique username for each.

Effective October 1, 2015, in accordance with CMS requirements, claims and prior authorizations with dates of service on or after October 1, 2015 must use the ICD-10 codes.

Avesis will no longer accept claims or prior authorization requests with the ICD-9 codes with dates of service on or after October 1, 2015.

For Dental Practices, Only

Effective October 1, 2015, Avesis:
• will accept and process all ICD-10 codes
• will not accept ICD-9 codes unless they are included in ICD-10
• does not currently require ICD-10 diagnostic codes
• will accept only the 2012 version of the ADA Claim Form

Claims submitted using prior claim form versions will be returned unprocessed.


ICD-10 for Kentucky Dentists

Kentucky dentists who currently do not bill ICD-9 codes do not have to convert to ICD-10. However, Kentucky Oral Surgeons currently billing ICD-9 codes will be required to comply with ICD-10.
