Important Updates

NPI update

All Health Care Providers were required to apply for a National Provider Identifier (NPI). The compliance date was May 23, 2007. This number will replace all legacy identification numbers including Medicaid and Medicare. The assigned NPI does not expire and contains no embedded information about the Health Care Provider that it identifies. Providers will not be able to submit electronic claims without an NPI number.

New Claim Form Instructions

Click here to find the new CMS 1500 claim form for Providers. Please review and ensure that you are filling the form out correctly when filing a claim. The NPI# for the billing provider should be entered in block 33a, which is located at the bottom right of the form. The facility’s NPI# should be entered in block 32a.
