Effective January 1, 2008 Avesis will administer dental and vision benefits to members of Care Improvement Plus. Enrollees are currently located in Arkansas, Georgia, Maryland, Missouri, South Carolina, and Texas.

Click here for a sample of the ID card (pdf) the members will present for services, along with a copy of the vision benefit design or the dental benefit design. Please review the member's benefit design and familiarize yourself with applicable co-pays, covered services, discount pricing, etc.

Please note: the Care Improvement Plus vision benefit has been revised for 2008!

You may choose from three convenient options to verify member eligibility:

• The Avesis Internet site anytime at www.avesis.com
• Integrated Voice Response (IVR) at 866-234-4806
• Calling Avesis Customer Service at 1-800-828-9341 (Customer Service Reps are available 7:00 A.M. to 5:00 P.M. MST, Monday through Friday)

Claim submission can be electronically submitted via our Internet site or mailed using a CMS/HCFA-1500 claim form.

You may access a tutorial video that will walk you through the steps for setting up your account, checking eligibility, submitting claims, checking the status of your claim, and various other functions online.
