Arizona Health Care Cost Containment System

Avesis Third Party Administrators, Inc. has partnered with Bridgeway Health Solutions to manage acute care services for the Arizona Health Care Cost Containment System. These services are designed for low income families with children, pregnant women, children under 18, and disabled people who are enrolled in Arizona's State Medicaid program.

This new plan will become effective October 1, 2008. Avesis will be paying claims for Bridgeway Health Solutions Members who reside in Yavapai County. Avesis is extending an invitation to all dental providers in Yavapai County or the surrounding areas to become providers in our network. Your participation will allow you to provide dental services for Acute Care Members in your community.

Avesis Third Party Administrators, Inc. has partnered with Bridgeway Health Solutions to manage dental services for Arizona Long Term Care System (ALTCS) Members. This program was implemented October 1, 2006. Avesis will be paying claims for Bridgeway Health Solutions Members who reside in Maricopa, Yuma, and La Paz counties.. If you are a provider in one of these three counties, we are inviting you to join our Avesis network of preferred Providers.

Becoming a provider couldn't be easier! Please complete the Provider Practice Application in its entirety and sign the Provider Agreement. All information should be mailed to Avesis at the address below. If you have any questions, please contact a Provider Services Representative at 800-327-4462.

If you are interested in participating in this new Medicaid program, you must have, or obtain, an AHCCCS Provider Identification number.
