Care Improvement Plus
As a DenteMax provider, Avesis will be utilizing your office to provide dental benefits to members. Avesis currently administers dental benefits for Care Improvement Plus Medicare Advantage members located in Arkansas, Georgia, Illinois, Indiana, Iowa, Missouri, New Mexico, New York, South Carolina, Texas, and Wisconsin.
Effective January 1, 2013, Avesis will be administering routine dental benefits for Care Improvement Plus Medicare Advantage Members residing in the following states: Alabama, North Carolina, Nebraska, and Pennsylvania.
Members' benefits vary according to their individual coverage. As a participating provider, it is imperative that you verify eligibility and benefits on all members each time they present in your office. Please remember that only the services covered by the member's benefit plan will be paid by Avesis.

Plan benefits include preventive services such as exams, cleanings and x-rays, simple restorative services, and may even include dentures and denture adjustments. Benefits and co-pays may vary depending on the member's plan. Some members may be eligible to receive a discount on dental services not already covered by the plan.
