Welborn Health Plan Vision Benefits
PLEASE NOTE: As of January 1, 2012 Avesis will NO longer be administering Vision and Dental benefits to Welborn members
As of January 1, 2009, Avesis administers dental benefits to members of Welborn Health plans. New and effective as of January 1, 2011, Avesis also administers vision benefits to members of Wellborn Health Plans.
Welborn members reside in the following areas of Indiana: Gibson County, Knox County, Pike County, Perry County, Posey County, Spencer County, Vanderburg County, Warrick County and surrounding areas.
