As of January 1, 2012 Avesis will be administering routine dental benefits to Molina Medicare Advantage members.

Below you will find a sample image of the ID card that members will present for services. Molina Medicare Advantage members will be in one of the following dental groups: Medicare Options, Medicare Options Plus, and Healthy Advantage.

Members' benefits vary according to their individual coverage. As a participating provider, it is imperative that you verify eligibility and benefits on all members each time they present in your office. Please remember that only the services covered by the member's benefit plan will be paid by Avesis.

Plan benefits include preventive services such as exams, cleanings and x-rays. Services that require prior authorizations are crown and bridge procedures and any treatment plan over $500. Please click on the link below to view specific plan information.
